Downeast Bilingual Community Health Worker – Year-round

Maine Mobile Health Program (MMHP) is hiring a Bilingual Community Health Worker to be based out of Milbridge, Maine. Maine Mobile Health Program is a community health center focused on supporting Maine’s farmworkers and seafood processors in accessing healthcare. MMHP aims to reduce health disparities and inequities of access for these communities by providing culturally appropriate care and services. The Community Health Worker: Core roles include conducting health/needs assessments, navigation, care coordination, interpretation, serving as a cultural broker, providing advocacy and health education.

What you will be doing:

  • Bridge the gap between communities and health and social service systems.
  • Conduct outreach and establish and maintain relationships with current and potential patients’ family members. Assess needs and identify appropriate resources and referrals.
  • Provide care coordination services on behalf of patients and their families, assisting with referrals and appointment coordination, interpretation, transportation, prescription assistance, and eligibility for insurance or public benefits.
  • Communicate with clients, staff, and partners in a respectful, culturally appropriate manner.
  • Provide intake, interpretation, or Provider Assistant services for mobile clinics as assigned.
  • Drive the Mobile Medical Unit as assigned.
  • Supervise other CHWs on the team by assigning care coordination activities
  • Support services related to COVID-19 including supporting and facilitating access to testing, vaccine, education and referrals to social services

What we will need from you:

  • Ability to speak Spanish
  • Current driver’s license, with good driving record
  • HS Education, GED, or equivalent experience
  • Daytime, and some evening and weekend hours during the harvest season
  • Ability to provide services in varied geography
  • Completion of a CHW core-competency training preferred
  • COVID-19 vaccination
