Maine Mobile Health Program (MMHP) is hiring a Bilingual Community Health Worker to be based out of Augusta, Maine. MMHP is a community health center focused on supporting Maine’s agricultural workers and their families through mobile health services and case management. The Community Health Worker is a respected member of their community who serves as a bridge between medical providers and individuals, promoting health and improving access to care. The core roles include conducting health/needs assessments, navigation, care coordination, interpretation, and health education.
Requirements of the Community Health Worker
- Ability to speak and write in Spanish
- Current driver’s license, with a good driving record
- Reliable transportation
- HS Education, GED, or equivalent experience
- Computer skills are required.
- Daytime, and some evening and weekend hours during the harvest season
- Highly motivated with strong interpersonal skills
- Strong understanding of the agricultural community due to prior work, relationships in/to the community, and/or other life experience such as shared language of culture.
- Ability to provide services in a varied geography
- Completion of a CHW core-competency training preferred
What you will be doing:

- Bridge the gap between communities and health and social service systems.
- Conduct outreach to establish and maintain relationships with current and potential patients.
- Provide care coordination services on behalf of patients and their families, assisting with referrals and appointment coordination, interpretation, transportation, prescription assistance, and eligibility for insurance or public benefits.
- Support CHW activities related to mental health and substance use including working directly with patients and other staff.
- Provide intake, interpretation, or Provider Assistant services for mobile clinics as assigned.
- Communicate with clients, staff, and partners in a respectful, inclusive, appropriate manner.
- Drive the Mobile Medical Unit as assigned.
